Due to the ongoing COVID- 19 pandemic, we are taking extra precautions to ensure the safety of our staff and clients. As such we are kindly ask you to fully complete and submit this form BEFORE your appointment with us.

YOUR APPOINTMENT
Please read each of the following statements carefully and tick to indicate that you agree

I agree to follow strict hygiene practices while at my appointment including washing or sanitising my hands on arrival, and exiting the Salon and will comply with staff instructions during my service.

I will not bring anyone with me to my appointment. It's strictly one on one only.

I agree to maintain physical distance from staff an other clients, I understand that our usual procedures will be different with respect to how you are greeted and how your service is delivered.

I understand that I will need to remain outside until mandatory sanitation has been completed to strict hygiene standards.

I agree not to bring food or drink items with me to consume, as all personal items such as, handbags, wallets, phones etc, will need to be placed in a provided plastic bag upon entry.

I acknowledge there will be no walk-ins for appointments, product or gift voucher purchases; these must be organised by prior phone arrangement only.

I agree to let you know if any of my details I have given change, particularly with regards to developing any symptoms of, or exposure to COVID-19.
